Base Control uses three related safety and compliance tools that you configure before members start using the portal: the health questionnaire (completed by members), waiver templates (signed by members), and incident recording (filed by staff). This article covers the admin configuration for all three.
This article covers product configuration only. For guidance on what to include in health questionnaires or waivers for your specific business, consult your legal adviser.
Health questionnaire
The health questionnaire is presented to members on their first login to the member portal. Members must complete it before accessing the rest of the portal.
Create a template
- Go to Settings in the sidebar, then select Health Questionnaire.
- Select Create Template in the top-right.
- In the drawer that opens, enter a Template Name (for example, Health & Fitness Questionnaire).
- Optionally set Expiry (months) — the number of months before members are prompted to re-complete the questionnaire. Leave blank for no expiry.
- Tick Set as default template if this template should be the one shown to new members.
Add questions
- In the template drawer, select Add Question.
- Enter the question text in the text field.
- Choose a question type from the dropdown: Free Text, Yes / No, Dropdown, Multi-Choice, or Date.
- For Dropdown or Multi-Choice types, enter the options as a comma-separated list (for example, Option 1, Option 2, Option 3).
- Tick Required to make the question mandatory before submission.
- For Yes / No questions, select + Add follow-up to show an additional question when the member answers "yes".
- Repeat for each question.
Reorder questions
Use the up and down chevron buttons beside each question to move it up or down in the form.
Preview the questionnaire
Select Preview on any template to see exactly how the form will appear to members, including the consent declaration and digital signature field.
Edit or archive a template
Select Edit on the template card to reopen the drawer. To hide a template from new use, select Archive — existing responses are retained. To reactivate an archived template, select Activate.
Waivers
Waivers are legal documents members sign before participating. You manage waiver templates from Waivers in the sidebar (not from Settings).
Create a waiver template
- Go to Waivers in the sidebar.
- Select Manage Templates, then Create new template.
- Enter a Template name and compose the waiver body using the rich text editor.
- Tick Required if members must sign this waiver before they can be checked in.
- Save the template. Members see it on their next login if it is unsigned.
Publish a new version
When the content of a waiver changes, create a new version so existing signatories are prompted to re-sign. Open the template in the Manage Templates panel and use the New version option. You can set a grace period (in days) and a re-sign option — either Notify (send reminders) or Block (prevent check-in until signed).
Send a reminder
From the waiver status table, locate a member with an Unsigned status and select Send reminder on their row to send an email prompt.
Incidents
Staff record incidents from the Incidents page in the sidebar. The incident types available are fixed system categories: Injury, Near Miss, Equipment Failure, Altercation, Medical Episode, Property Damage, and Other.
There is no additional configuration required to start recording incidents. To understand the full incident lifecycle (open → reviewed → closed), see Create and manage incidents.
