When you first sign in to the member portal, you're asked to complete a short health questionnaire before you can go further. Here's how.
Fill it in
- Answer the health questions on the Health Questionnaire page.
- Read the Consent Declaration and tick I consent to this information being shared with relevant instructors.
- Under Digital Signature (Type Full Name), type your full name to sign.
- Select Submit Health Questionnaire.
You can't submit without ticking consent and typing your name — both are required.
After you submit
Once submitted, the rest of the member portal unlocks and your instructors can see the information relevant to keeping your training safe.
